An island needs clear walkways on every side, and in a unit of a few hundred square feet that floor usually works harder as a peninsula, a counter that juts from the wall and seats 2 on stools. We sketch both versions on the spot so the choice gets made with real numbers.
Island or peninsula
An island suits a room wide enough to walk around it without turning sideways. A small ADU seldom has that width, so a peninsula gives more counter for the floor it takes. It anchors to the wall, can carry the sink or the cooktop, and seats people on the outside edge. Outlets on its end panel satisfy code for counter receptacles. If the unit is wider, an island becomes possible, though plumbing in it means a drain line through the slab, which we plan at the foundation stage.
Alhambra permits on an urban lot
Alhambra's plan check covers setbacks, height, egress and parking where it applies. A backyard unit on an urban lot may sit close to neighboring buildings, so a wall within 5 feet of a property line needs fire-rated construction and openings in it are limited. State law also bars a demand for replacement parking when a garage becomes the unit. We place windows for privacy and light, then run the kitchen exhaust out a wall that faces away from the neighbor.

From the main house, if the lot allows it. A new water branch carries its own valve, waste leaves the unit by gravity for the existing sewer lateral, and the electrical feed lands in a subpanel inside. If the house panel cannot take the extra load, a separate meter for the unit is the other path.
